Sign up and Membership Plans
How to sign up
Signing up is easy. Just print, review and sign the three require documents; the Membership Agreement, the Physical Activity Readiness Questionnaire, and the ACH agreement. Then, contact us by either phone or email to schedule an appointment. If you don’t have a printer, don’t worry. We have plenty of copies of our forms and will meet you with them. Please bring your ID and voided check from the account you wish to use for your monthly ACH draft. Or if paying for an entire year, please bring a check for $300 plus tax per person. ($325.13)
Our paperwork is below. Just click on the links and review or print for signing. You will need one of two mebership agreements; if you are going month-to-month, please select the Month-to-Month Membership Agreement and if you are paying in advance, please use the Paid-in-Full Membership Agreement.
You will need one of either membership agreement and a PARQ filled out. If you are using the Month-to-Month Membership Agreement, please fill out the ACH Agreement and bring in a voided check.
